Data informed Stepped Care (DiSC) Study The goal of this study is to conduct formative work toward development of a data-driven, health systems intervention to improve engagement in care and clinical HIV outcomes among adolescents and young adults (AYA) in Kenya. Specifically, we will determine the feasibility of establishing a cohort, develop and validate intervention tools, and ensure a comprehensive understanding of ethical issues.
Aim 1:To establish a clinic-based prospective cohort of HIV-positive AYA ages 10-24 and their caregivers in Western Kenya and evaluate key correlates of poor retention, poor adherence, and virologic failure.
                        Aim 2: To develop and validate an age- and developmental-stage specific clinical prediction tool that identifies AYA at risk of loss to follow-up (LTFU) needing more intensive support interventions.
            Aim 3: To optimize practices for adolescent consent and for transitioning from caregiver-supported to independent consent in HIV care and research
